Certificate of Disablement disappeared from NIS office

I lost a thumb and two index fingers on my left hand and I am living overseas and have authorised my sister to uplift my money from the National Insurance Scheme. I sent my Life Certificate for Disablement since March 31, 2000 to my sister who usually takes it in to NIS, and within two weeks she would get the money.

On February 12, 2009 my sister took in the certificate and was told to throw it in the box and to return in the first week of March, which she did; she then was told to go back in the last week in March, which she did, but was told to return on April 17, which she did, only to be told that the certificate was not lodged and she has to get another certificate.

Every time I have to get another certificate, I have to wait until my day off and pay $US30 to a Notary Commissioner to stamp it. Could someone please help us?
